Stratford Connection Centre and Street Outreach
The Stratford Connection Centre and Street Outreach program provide wrap around supports to individuals experiencing homelessness, those who are accessing emergency shelter, individuals who are precariously housed, or who are otherwise vulnerable by connecting them to housing, medical, financial and legal services and supports. Services and supports are administered by trained professionals and include trauma informed care, substance use, and mental health supports.

The Stratford Connection Centre is operated by CMHA Huron Perth Addiction and Mental Health Services and is located at St Paul’s Church, 9 Douro St., Stratford
What services does the Stratford Connection Centre provide?
The program provides case management and housing stability support to those in Stratford and Perth County who are experiencing homelessness or who are precariously housed and otherwise vulnerable.
How and when can the Stratford Connection Centre be accessed?

Northern Huron Connection Centre
The Northern Huron Connection Centre provides wrap around supports to individuals experiencing homelessness, those who are accessing emergency shelter, individuals who are precariously housed, or who are otherwise vulnerable by connecting them to housing, medical, financial and legal services and supports. Services and supports are administered by trained professionals and include trauma informed care, substance use, and mental health supports.
