Heart to Home Bridge Housing and Stability Program

The Bridge Housing Program is an innovative program that aims to transition vulnerable individuals out of homelessness into temporary living spaces that offer stability and support. Utilizing a “Dispersed Model of Supportive Housing” individuals will be accommodated in units throughout the community, providing a safe environment for individuals needing rest, food, and connection to appropriate services.

In addition to the Bridge Housing Program, Heart to Home also provides wrap around supports that are provided by trained professionals including trauma informed care, substance use, and mental health supports. The goal of Heart to Home is to transition those accessing the Bridge Housing Program, experiencing homelessness or are precariously housed into permanent housing solutions. The Bridge Housing Program can support a limited number and can accommodate up to 14 individuals. The team works from a housing first model providing support and ongoing intensive case management support in collaboration with the County of Huron.

The Bridge Housing Program welcomes those who are experiencing homelessness to contact them for further conversation and intake.

What services does Heart to Home provide?

The program provides intensive case management support to those in Huron County who are experiencing homelessness or are precariously housed.

How and when can Heart to Home be accessed?

To access the Bridge Housing Program you can call 519-525-5922. Intake occurs Monday to Friday between 9am and 5pm.

The program is available for Huron County residents only.
